It only becomes a problem if the web server resolves mixed-case requests differently.

For example, if your server treats the URL site.com/PAGE.HTML differently from site.com/page.html then you will have an issue. However, as EPiServer runs on IIS then this is not a problem as the URL will resolve to the same page no matter what the case is.

Two quick comments:
1. We will fix this for the next version of CMS - lowercase will be the default.
2. I recommend that you detach from the event handler in the Uninitialize method. In the common runtime scenario it is not an issue, but if you want to run integration tests you may have multiple Inintialize/Uninitialize calls.

In your image it seems this works from top to bottom, however for any user used to working with any Adobe or other professional graphics package they would be used to work from bottom to top.

Of course this is an issue in the fact that Umbraco inserts new content at the bottom of the list, however I could think of numourous live production projects we have where it would be considerably more applicable if the content was populated into the top of the list.

Maybe a function to control this in Umbraco would be a worthwhile cause?
